This is due to the anatomical configuration of the talus with the talar dome wider anteriorly and narrower posteriorly, so that when the ankle is plantar flexed, the narrower posterior portion of the talus is brought into the fixed mortise (tibial side) giving wider medial clear space. This resembles a "pose hack" photo! So, proper imaging technique is crucial.
