A very common diagnosis for shoulder injuries is a superior labral tear, or SLAP tear. SLAP stands for Superior Labral tear Anterior to Posterior. As you can see in the figure, the long head of the biceps tendon inserts directly into the superior labrum. A SLAP tear occurs when the labral is injured at this area. There are several variations of injuries that can occur to the superior labrum where the biceps anchor attaches, but the biceps tends to create either a peel-back or traction injury to the labrum. Peel back lesions tend to happen in overhead athletes with full ER in the layback position. Traction injures can occur in anyone with a forceful distraction of the arm and eccentric contraction of the biceps.
